How to acclimatise for a drive to Ladakh: the 48 hour rule, explained

Ladakh's own advisory makes 48 hours of rest in Leh compulsory before you go higher, and it says road travel does not count as acclimatisation.

If you are driving or flying into Leh, the official answer is simple. The Leh district administration requires every tourist to spend at least 48 hours acclimatising before heading to higher ground like Khardung La or Pangong. You can potter around Leh town in that window. You cannot point the nose at a 5,000 metre pass on day one.

The part most owners get wrong is the next line in the same official health advisory: "Road travel does not help at all in acclimatization. It builds up the altitude effect progressively." So the common belief that a Manali to Leh or Srinagar to Leh drive counts as acclimatisation does not match what the authority says. If you drove up over Baralacha La and Nakee La, you have already spent hours above Leh's own height. The 48 hour clock still starts when you reach Leh.

What the official schedule actually looks like

Ladakh Tourism's advisory lays out a day by day plan. Day one is complete rest, no sightseeing, light meals and fluids. Day two is short walks in Leh town only. Day three is nearby attractions at a similar altitude. From day four you go higher, and the advisory caps the gain at 500 metres a day. It also asks for 3 to 4 litres of water daily, no alcohol in the first 48 hours, and Diamox only as prescribed. The permit portal's version adds that there should be no active physical exertion on the first two days.

Learn the two symptom lists, because they decide whether you keep driving or turn around. Mild acute mountain sickness is a persistent headache, mild nausea, poor sleep, low appetite, slight dizziness and fatigue. The serious ones are a worsening headache that medicine does not touch, repeated vomiting, confusion, loss of coordination, breathlessness at rest, a cough with frothy or pink sputum and blurred vision. Those last ones point at HAPE or HACE, and the only real treatment is losing altitude fast. On a Ladakh trip your vehicle is the ambulance, so always know which way is downhill and how far. Keep the Leh numbers saved: SNM Hospital on 01982-252014 and the Chief Medical Officer on 9419811156. Portable oxygen is sold at some hotels and pharmacies in Leh, and it buys you time, not a cure.

Your engine is acclimatising too

Thin air costs power. Garrett Motion's own explainer puts the rule of thumb for a naturally aspirated petrol engine at 3 per cent of power lost for every 1,000 feet of elevation, so a 100 hp engine makes about 85 hp at 5,000 feet and 55 hp at 15,000 feet. A turbo compensates by compressing more air, which is why a modern turbo-diesel Thar, Jimny petrol-turbo or Fortuner feels far less strangled at Tanglang La than an older naturally aspirated engine does.

Plan for it. Use lower gears and higher revs, expect longer overtakes, and do not load the roof rack as if you were on a plains highway.

Before you leave, and in winter

Check IMD's Meteorological Centre Leh page for the district forecast, district-wise warnings and nowcast alerts rather than a phone weather app, and read them the night before and the morning of a pass crossing. Ladakh's advisory warns that winter temperatures can drop to -30 degrees Celsius and asks for cold-weather gear and emergency supplies on any trip between October and March. The advisory's own words are the best summary for a driver: do not ignore weather advisories when planning passes.
